Identifying the Problems
We sit down to meet with you and your loved one in their home. We ask many specific questions that uncover their day to day needs. Some examples of our questions include:

The answers tell us about what’s working and uncover the problems that need solutions. For example, increasing needs creates increasing costs. We explore Medicare, Medicaid and VA benefits to lessen the cost of caregiving.
The Solution
After meeting with and understanding your loved one we come up with a personalized solution. It will take into consideration where and how your loved one lives and what matters to him or her for the short-term and long-term future.

Going Foward
A Caring Choices care manager will:

coordinate, communicate with the health care team and be your advocate for the care of your loved one, whether in the home, in an assisted living or nursing home or during a hospital stay.

accompany your loved one to doctor’s appointments to assure that the doctor receives the right information and the doctor’s orders are understood and carried through. The outcome is communicated back to you via phone or email.

conduct ongoing home visits assure that all the needs are met as changes occur in health and behavior.
